Strengthening Sepsis Care: Collaborative Insights into Hour-1 Bundle Implementation in Indonesia
A recent multicenter qualitative study, published in Acute and Critical Care (2024), explores the ongoing challenges in implementing the Hour-1 Sepsis Bundle in Indonesian secondary referral hospitals. Researchers from six institutions, Universitas Medika Suherman, Universitas Ichsan Satya, Universitas Muhammadiyah Semarang, Universitas Esa Unggul, Universitas Padjadjaran, and Universitas Brawijaya, gathered perspectives from emergency and intensive care unit healthcare providers through in-depth interviews.
The study reveals that despite recognition of the Hour-1 Sepsis Bundle as a critical protocol, its consistent application remains limited. Healthcare providers experience delays in diagnosis, largely due to unavailable or delayed blood culture testing and limited access to clinical scoring tools such as qSOFA and SOFA. In addition, collaboration among medical staff is often fragmented, with insufficient communication and shared decision-making between doctors and nurses. Challenges in treatment delivery, including medication shortages and gaps in staff training, further impede timely sepsis management.
The researchers emphasize the necessity to adapt clinical guidelines to the local context, enhance health worker roles and training, and strengthen leadership support within hospitals. These measures aim to reduce preventable sepsis deaths and improve patient outcomes in resource-constrained environments.
This research supports Sustainable Development Goal 3 (Good Health and Well-being) by targeting improvements in emergency care and sepsis management. At the same time, it exemplifies SDG 17 (Partnerships for the Goals) through collaboration across multiple Indonesian academic institutions, demonstrating the value of partnership in enhancing healthcare quality.
