Faculty of Nursing Universitas Padjadjaran Explores Implementation Science to Enhance Discharge Planning
Effective discharge planning is essential for maintaining continuity of care after hospital stays. Researchers from Universitas Padjadjaran’s Faculty of Nursing, led by Urip Rahayu, S.Kp.,M.Kep., Prof. Kusman Ibrahim,S.Kp.,MNS.,Ph.D., and in collaboration with Hasan Sadikin General Hospital, Herry Herman, dr., SpOT., PhD., have systematically reviewed how Normalization Process Theory (NPT) is applied to improve discharge planning interventions.
Their study synthesizes evidence showing that NPT facilitates better coordination among healthcare teams, strengthens psychosocial support for patients’ families, and increases patient compliance with post-discharge treatments. Using NPT as a framework helps healthcare providers understand and overcome barriers to implementing discharge planning effectively.
This work highlights the Faculty of Nursing Unpad’s leadership in integrating theory into practical healthcare improvements, supporting Sustainable Development Goal 3 (Good Health and Well-being).
